My team is not learning from previous mistakes and we repeat the same mistakes, rather than learn and change what is needed.
Unfortunately, since I am on that team I can see that there is a churning. I am a manager who believes that making a mistake is good as you learn from them, but I need to work on a constructive behaviour with my team.
Any ideas are appreciated from where to start with and how to build the whole idea to properly learn from mistakes and not be afraid to make decisions.
Build an error log (or Issue Log), in which people from each team record all their mistakes and bad outcomes so then you could track them and address them systematically.
This will also play as a library and mistakes will not be repeated, but new mistakes will be logged.
