All,
I would like to recommend to my company a software or a more innovative way for creating Automated Bribery Risk Assessment Surveys. It should be a user-friendly, self-directed assessment to help compliance.
It should also be a way to extract the information for the reports in an easy and consistent way.
Let me know do you have any experience with similar software implemented.

That is a good challenge. My recommendation is the following:
1. Try Neota Logic https://www.neotalogic.com
Users will be able to respond to series of questions and then the tool provides the answers , collected in a report to the Compliance team. The good thing is that you can schedule the data to be automatically emailed to the team. In addition to that, you can create follow up questions to be asked based on how you have responded to the previous questions. At the end of the survey, the application uses the answers of each individual and generates assessment that is automatically emailed to the Compliance team. Neota is also a good as you can upload different files.
2. Similar to Neota Logic is HighQ.
The two softwares are allowing the user to fill a questionnaire more efficiently and to focus on answers that are applicable for his/her only.
